If your child is unable to attend school due to a documented medical reason for twenty (20) days or more, you may request that your child receive home instruction. If you choose for your child to receive home instruction, he or she will be enrolled in Home/Hospital School, San Diego Unified and be dis-enrolled from his or her regular school of attendance. When your child is able to return to school, the parent/guardian will need to re-enroll the child in the regular school of attendance.
Students who receive home instruction receive up to one hour per day of educational services from a certificated teacher. Every effort is made to maintain continuity in the course of study in the core academic areas. If a student receives special education services, the student will receive supports that are consistent with his or her Individualized Education Program (IEP). Home instruction is intended to be a temporary placement and is considered to be a highly restrictive setting.
Eligibility
The student's illness or injury must be diagnosed and documented in writing by a physician or psychiatrist/licensed clinical psychologist. Verification of student's medical condition, as well as information related to the medical condition, may be required.
Requesting Homebound Instruction Services
To request educational services in the home for your child who is medically homebound, parent/guardian and physician should complete and submit a Request for Homebound Instruction Packet.
