HOSPITAL INSTRUCTION

Overview
Educational services are provided to a student with a temporary disability admitted to a local hospital or other health facility located within San Diego Unified boundaries (California Education Code Section 48207). The purpose of hospital instruction is to support continuity of learning when a student is hospitalized. Instruction is provided by a certificated teacher who will work with your child for up to five hours per week.

Eligibility
If your child is hospitalized in a hospital or other health facility within San Diego Unified School District boundaries, you may request that your child be enrolled in San Diego Unified and receive instruction while in the hospital. Physician approval is required.

If you choose for your child to receive instruction in the hospital, he or she will be enrolled in Home/Hospital and Transition Supports School in San Diego Unified School District and be dis-enrolled from his or her regular school of attendance. When your child is able to return to school, the parent/guardian will need to re-enroll him or her. The teacher is available to support your child's transition back to a regular school setting.

How to Request Hospital Instruction for Your Child
The parent/guardian and the child's physician must complete the Hospital Instruction Request Form (Spanish version). Upon receipt of the completed request, the educational liaison will contact the parent/guardian.
